1. Category Of Applicant

   1. Owner of the content; or.
   2. Any authorised person by a written approval from the owner of the content.

2. How To Apply

   1. Application is made by using Borang ODL-1. This form is free and can be obtained from:-
      • Enforcement Division, Ministry of Domestic Trade and Consumer Affairs , No 13, Level 3, Persiaran Perdana, Precint 2, 62623 Putrajaya; or
      • State and district KPDNHEP offices throughout the country; or
      • Download from www.kpdnhep.gov.my
   2. Each application must be supported with the following documents:-
      1. Statutory declaration certified by a judge, magistrate or Commissioner of Oath;
      2. Related document of rights ownership such as letter of authorisation or agreement. Each copy must be certified as a true copy by a judge, magistrate or commissioner of oath;
      3. Proof of purchase order or purchase of optical discs consisting the title applied;
      4. Sample of inlay; and
      5. Other related document:-
         • A copy of Business or company or association registration certificate
         • A copy of National Registration Identification Card for individual application
         • Approval from the Ministry of Home Affairs i.e. Certificate A for films, Al-Quran screening approval and compliance with the issued Guidelines.
   3. The complete application may be submitted to the Enforcement Division, Ministry of Domestic Trade and Consumer Affairs , Putrajaya or to any state and district KPDNHEP office.

3. Maintenance Of Record

   Every person to whom an approval for optical disc label is granted must keep and maintain a proper record of the use of the optical disc label and other information in the ODL-3 Form in the Fourth Schedule and the record must be;

   1. Tavailable at all times for examination by the Controller;
   2. kept up to date and in good order and condition; and
   3. submitted upon request by the Controller.

4. Approval

   • The complete application will be processed and approved within 5 working days from the date of receipt
   • The Controller may approve or reject any applications for optical disc label
   • The Controller may impose any condition deemed necessary

5. Payment

   • Price of Optical Disc Label is RM0.163 per unit.
   • Payments can be made in the name of Ketua Setiausaha, Kementerian Perdagangan Dalam Negeri dan Hal Ehwal Pengguna in the form of:
     1. Bank Draft
     2. Money Order
     3. Postal Order
